Patient Portal and Forms

When you set up your first appointment, you will receive an email invitation to log in to your secure Potomac Therapy Group patient portal. The required intake forms will be posted to your portal for you to complete online. Please complete these forms at least 24 hours in advance of your first session so that your therapist may have time to review your information. The intake forms include practice policies and procedures and important documents to complete in full before your first session. Potomac Therapy Group is a fully paperless practice. During your first session, your therapist will be glad to answer any questions you may have about the forms.

Your Superbills will also be posted monthly to your Potomac Therapy Group patient portal.

YOUR FIRST SESSION

Free parking is available.  Our suite is number 540, which is located on the 5th floor of the building.  When you arrive-- please help yourself to a beverage, have a seat in the waiting room, and your therapist will come get you at the appointment time.

We respectfully ask that clients and their families be mindful of noise levels while sitting in the waiting room.  Please silence your devices and take phone calls outside to preserve a quiet space for therapy sessions.

Please note there is a live-stream security camera on the premises​.  The live-stream camera is for security purposes only at the suite entry.  The camera does not record or store identifying footage of visitors and there are no cameras in the therapy rooms.​ The camera captures video only and does not capture and/or record any audio, so that your privacy can be respected while ensuring the security of the office.

PAYMENT POLICY

Payment is expected at the time of service.  Your credit card on file with the practice will be charged on the day of your scheduled appointment.  All billing and financial information is stored electronically in a secure HIPAA-compliant manner.  

CANCELLATION POLICY

Please give a minimum of 24-hour advance notice for any appointment cancellations.   If you miss an appointment without giving at least 24-hour notice, you will be charged the full session fee.  In cases of inclement weather, your therapist will attempt to contact clients who have scheduled appointments and decide jointly if the appointment will stand.  The 24-hour notice policy does not apply to cancellations due to inclement weather.

IN THE EVENT OF AN EMERGENCY

Due to the nature of the profession, your therapist is often not immediately available by telephone.  At those times, you may leave a confidential voice message at the following number:  (301) 852-5500.  Your therapist can also be reached by e- mail.

In the case of a life-threatening or psychiatric emergency, please call 911 or go to the nearest hospital emergency room and ask to speak with a psychiatrist.
